Entry Forms shall contain the following:

A. The words: “Held under the current SCCA Time Trials Rules and regulations.”

B. The SCCA sanction number assigned to the event (a placeholder may be added before the sanction number is issued; e.g. ##-TT-####.) 

C.A. Full names, addresses, membership numbers, Region of Record, and license of entrants and drivers (for online registration this information is often part of their profile and may not need separate questions).

D.B. Full description of cars to be entered (year, make, model, trim level).

E.D. Acknowledgement of the following declaration, captured either by a physical signature on a paper form or checkbox confirmation in online registration platforms:

1. 1. 1. The Declaration of Medical Fitness physical ability statement, “I understand that participation in any capacity in motorsports is dangerous. Known or unknown conditions and situations involving myself or others, including but not limited to vehicle preparation, equipment condition, facility and environment, physical fitness, medical conditions, medications, knowledge, abilities, or skill can cause injury (significant or otherwise) and death. I understand that I cannot participate in the event if I am on medication or have a medical condition that impacts my ability to participate. My decision to enter an event in any capacity, and to continue my participation throughout the event, is voluntary and with the knowledge of the above conditions and potential outcomes.
         
         
      2. By submitting this entry, I agree to comply with the current SCCA Time Trials Rules as amended by Fastrack, including all applicable event entry standards, waiver provisions, and media rights policies. I understand that participation in this event requires adherence to these rules and procedures, and I affirm my commitment to upholding SCCA’s values and fostering a welcoming, respectful, and inclusive environment throughout the event.
         
         "I certify I have the ability to handle the physical and mental demands of competition for the length of a session for each session of the event." The page describing the requirements can be found HERE, and should be linked whenever possible.

G.F. Emergency contact information for the driver must be included with the entry.
